While browsing through the various stock photos on DA I found these two wonderful pics and Johnny the Homicidal Maniac instantly popped into my head, thus the quote and design of the fan art. The background was compiled in PSP with brushes, took about an hour to complete.

I like the pin-knee x-ray, that looks nice and painful. This almost looks like a blood-spattered police profile, rumpled up and thrown into a cellar where it was left to decay. I like the message in it as well. The mood is obviously conveyed, however it does seem a little bit abstract. Maybe there's something you could do to make it a little more specific, perhaps, in terms of an implied narrative or context? I mean, it's fine as it is, just to look at. But you could make it more. :)
